FAP's HIV Family case management services are designed to maintain, enhance
or promote family stability and the functional integrity of the family unit.
FAP defines a family unit as including at least one child and one adult parent
or guardian. To be eligible for Family Support services, at least one member
of the family must be living with HIV/AIDS.

FAMILY SUPPORT SERVICES
- Intake and comprehensive assessment of the family's available resources and needs
- Development and implementation of the family service plan
- Coordination of services required to implement the family service plan
- Interventions, linked referrals, monitoring and follow-up
- Periodic reassessments of the family's status and needs
