POMONA HOMELESSNESS PREVENTION

Homelessness increases the risk for contracting HIV.  In partnership with the City of Pomona, FAP’s Homeless Street Outreach Team visits locations throughout the city to provide referrals, transportation, food and housing assistance for individuals and families who are homeless and in danger of becoming homeless, regardless of their HIV status. Services include:

  • Screening/intake
  • Emergency housing
  • Transportation
  • Referrals to benefits and supportive services
