Case managers coordinate and arrange transportation for registered clients to
medical appointments, pharmacies, and social service appointments.
Clients enrolled in FAP's Case Management Program with a household income less
than 200% of the federal poverty guidelines are eligible for transportation
services. Eligibility will be verified by documentation of household income.

FAP has agreements to provide vouchers for the following agencies to meet
clients' transportation needs:
- Independent Taxi Operators Association (ITOA)
- OmniTrans (San Bernardino)
