| HOUSING |
Housing Case Managers work with clients to develop an individualized housing plan and a monthly budget that supports independent living in a stable living environment. Clients seeking HOPWA assistance must meet basic eligibility requirements determined in an initial assessment. Contact the Housing Department to see if you qualify.

| TRANSPORTATION |
Transportation to and from medical and service appointments is available to registered clients and varies by County. |

| FOOD ASSISTANCE |
Monthly food assistance is available for registered clients, and is based upon eligibility and availability. |

| HEALTH EDUCATION & OUTREACH |
Since 1998, FAP has provided San Bernardino County-funded HIV prevention/education and outreach services. Services are provided at non-clinical settings and public spaces where the target populations congregate. |

| POMONA HOMELESS PREVENTION |
Homelessness increases the risk for contracting HIV. In partnership with the City of Pomona, FAP’s Homeless Street Outreach Team visits locations throughout the city to provide referrals, transportation, food and housing assistance for individuals and families who are homeless and in danger of becoming homeless, regardless of their HIV status. |
